Request Personal Care Management Services
for yourself or someone else
Are you enrolled in MassHealth Standard, CommonHealth or a OneCare Plan?
Do you require hands-on assistance with at least 2 activities of daily living (such as bathing, toileting, transferring, eating)?
Do you or an approved appointee want to be in charge of directing your own personal care?
If you answered YES to the questions above, you may be eligible for Your Access Advocates’ Personal Care Attendant (PCA) Program.
Request Services for Independent Living, Deaf & Hard of Hearing Independent Living, or Transition to Adulthood
Your Access Advocates helps people with disabilities learn to live more independently. Independent Living (IL) Skills Trainers provide one-to-one services to individuals throughout Central Massachusetts. The five core services are Information and Referral, Peer Support, Skills Training, Advocacy, and Transition.
Pre-Employment Transition Services
Pre-Employment Transition programming is intended to emphasize vocational and educational awareness, exploration and preparation. The goal of Pre-ETS (Pre-Employment Transition) is to provide students with skills for employment or education after high school.
