One Care Long Term Supports & Services (LTSS)

One Care is a Self-Directed Health Care model for individuals enrolled in MassHealth and Medicare. The goal of this program is to provide a less expensive, better coordinated health care with greater access to necessary supports. Your Access Advocates Independent Living LTSS Coordinators work with individuals to establish the supports necessary to improve health. The intent is by accessing appropriate supports services individuals will be supported to living independently in the community in which they live.
